How one hospital built an interdisciplinary high-utilizer program around individualized care plans embedded in the EMR. Six months post-intervention: a 56 percent reduction in ED visits, a 67 percent reduction in inpatient admissions and a 56 percent reduction in radiology studies. Includes the phased implementation approach, compliance and EMR build considerations, and how to adapt the model to your own resources.
