During this session, care management components of MDPCP will be discussed with a concentration on HEART payment. Best practices for proactive outreach, assessment of unmet social needs, and care coordination will be shared.

ABSTRACT: Discharge planning and Care Coordination are an important aspect of improving the quality of life, including preventing untoward events following hospitalization. The prevalence of people suffering from multiple chronic conditions poses a significant risk when they are not properly identified. These patients also suffer from poor quality of life and increased mortality risk. Early identification and involving other key stakeholders, particularly high-risk patients, helps eliminate barriers to discharge planning and provides an avenue for successful Coordination of Care.

- Demonstrate an understanding of the Cornerstone of Discharge Planning
- Develop a strategic approach when managing complex cases
