Kerrie supports individuals and families in North Sydney to navigate aged care. She will share her perspectives on emerging Sydney‑wide trends that are placing growing pressure on an already stretched system.

Nicholas Crowe (Hello HealthCare): Preventing the 'Revolving Door' of Hospital Readmission
Nicholas provides community-based rehabilitation across Sydney and works in partnership with healthcare professionals to support safe transitions home and reduce the risk of readmission. He will explore how early in-home physiotherapy is one of the simplest and most effective ways to reduce avoidable readmissions.
