WHAT IS FAMILY-BASED FAITH FORMATION?
In Family-Based Faith Formation, Parents teach their children at home with the same textbook and materials used for In-Person Faith Formation Classes.  Parents are provided with the textbook and lesson plan for the year for their child(ren)'s grade(s).  Parents set aside time each week to teach their children the lessons at home and then submit their children's assignments to a Home-based Catechist, who tracks the students' progress in the program.  Attendance is also required at the bi-monthly Feed Your Faith events as part of the Home-based Faith Formation Program (4 total per year). All Feed Your Faith events include dinner for the whole family. Because of the added expense of providing a family dinner, the fees for Home-Base Faith Formation are slightly higher than the fees for In-Person Faith Formation Classes. All Parents registering their children for Family-Based Faith Formation will be asked to sign a pledge stating that:

1. You will use the lesson plans provided.
2. You will set aside time each week to teach each lesson to your child/children.
3. Your child/children will submit assignments to our Family Based Catechist in a timely manner.
4. Your family will participate in the bi-monthly "Feed Your Faith" Family Nights (4 total throughout the year). Participation is required.

If your family does not fulfill these requirements, the Faith Formation Year may not be considered complete. If your child receives an "incomplete" for the Faith Formation year, your family may not be able to participate in the Family Based format the following year. Confirmation of enrollment in Family-Based Faith Formation for the current year is pending approval.

All families registering children and youth in Faith Formation must be registered active parishioners of Most Precious Blood.

To prepare for Sacraments, students must be enrolled in Faith Formation and Sacramental Preparation. Registration for Sacramental Preparation (First Reconciliation/First Communion and Confirmation) is a separate enrollment and in addition to Faith Formation registration. Enrollment for all Sacraments takes place in person at the Mandatory Faith Formation Parent Meeting held during the first Faith Formation Class of the year (Sept. 1st and 2nd; Sept. 3rd for Family-based Families). Enrollment forms for Sacramental Prep. are NOT available online.
