Join us for a day of learning and collaboration to promote successful use of Supported Living, an NC Innovations Waiver service for people with intellectual/developmental disabilities (I/DD).

Defined by NC Medicaid, Supported Living provides a flexible partnership that enables a NC Innovations beneficiary to live in their own home with support from an agency that provides individualized assistance in a home that is under the control and responsibility of the beneficiary (NC Medicaid: North Carolina Innovations, CCP 8P, May 2022).

This conference will offer an opportunity to learn more about the best-practice models being implemented by I/DD providers across the state. Key-note speakers will address the importance and relevance of the Supported Living service, how it increases personal outcomes for people who use it, and the efforts of Stakeholders state-wide to continue the successful promotion and improvement of Supported Living for ALL.

People receiving I/DD services or interested in services, family members, provider agencies, MCO/LME, state-employees, community stakeholders, and other interested community members are invited to attend.

This event is FREE to attend. It is sponsored by Liberty Corner Enterprises, Incorporated and the Supported Living 2 & 3 How-To Guidebook initiative in collaboration with North Carolina Council on Developmental Disabilities.

Key Note Speaker - Steve Strom
Steve Strom is the Director of Money Follows the Person with the NC Department of Health and Human Services
He has been with the MFP Demonstration Project since 2017.  Prior to becoming the MFP Demonstration Project director, Steve was a Systems Change Manager for the North Carolina Council on Developmental Disabilities in Raleigh and a frequent speaker at disability related events. He managed a portfolio of grants for the Council totaling over $3,000,000 supporting innovation for services for NC citizens with intellectual and other developmental disabilities focusing on community transition, natural supports, employment, guardianship reform, creating medical health home models for patients with I/DD, aging caregiver support, building self-advocate and family leaders during a time of change in Medicaid, and led the largest funded project to date for the Council on raising expectations for students as they transition from high school to adulthood.  Steve is a huge champion of Supported Living and promoting the success of people living with intellectual/developmental disabilities across the state of North Carolina. Steve will share highlights of his work contributing to enhancing the use of Supported Living and a first-hand perspective of unique initiatives the state is funding to achieve successful home and community based services in NC.

Key Note Speaker, Linda Kendall Fields M.Ed.
Linda Kendall Fields is a Clinical Associate Professor at the UNC School of Social Work in Chapel Hill. She has dedicated over 35 years to building communities that are responsive to the needs and contributions of older adults, people with disabilities and families in Minnesota, Oregon, Ohio, Georgia, and North Carolina.
In her capacity at UNC Chapel Hill, and as coordinator of the Cares Program, she has been leading several initiatives aimed at increasing greater choice and self-determination through systems change. This includes the statewide Rethinking Guardianship initiative, Money Follows the Person Building Capacity grants; and the Supported Living 2&3 Action Team.
Linda will share her experiential knowledge of the work being done to support people living with intellectual/developmental disabilities to live in their own homes with success! Linda is masterful at facilitation, project management, and educating the stakeholder community on best-practices in home and community based services.

Mary Beth Clemons, Person-Centered Practices Mentor Trainer, The Learning Community for Person-Centered Practices. 
Mary Beth has been a PCP Mentor Trainer for more than twenty years. She dedicated over 25 years as a Programmatic Director at Liberty Corner Enterprises and has continued as a consulting capacity providing person-centered training and assistance to people and their support teams across the nation. 
Mary Beth will lead a workshop informational session sharing tips, tricks, and tools to spark creativity when planning for how best to support people to live their best lives.
 
  1. Addressing Housing & Transportation Needs in your community!
Paul Moon, Land of Sky, Regional Planner/Project Manager
Paul is passionate about helping people who are under-served and populations who face barriers to important resources to live in their own home with success. Paul is the project manager of the WISH grant, funded by NCDHHS Money Follows the Person, focusing on accessing housing and transportation which is affordable for people with fixed incomes and scarce resources. He will share what he has learned about how to identify and connect to resources in your unique communities.
 
  1. Using Technology Supports with success!
Ayelet Heckathorn, Director of Home and Community Based Supports, The Charles Lea Center. 
Ayelet has 20 years of working experience supporting people with intellectual and developmental disabilities, ranging from early intervention services to working with seniors.  She moved to North Carolina from Pittsburgh, PA in 2016, where she spent 2 years working in Care Coordination for Trillium Health Resources.  It was in that role where she saw a presentation about The Charles Lea Center in South Carolina, which piqued her interest in the creative ideas and approaches they had to supporting people and providing services.  Ayelet began her career at The Charles Lea Center in 2018, where she was hired to develop, create, and implement Supported Living services in the Wilmington and Greenville areas.  
Ayelet is a cheerleader and champion for the people The Charles Lea Center supports.  She will share her strong belief in peoples’ abilities, voices, and values to direct and guide their support, above all else.  She will also share her work with the team at The Charles Lea Center to develop truly person centered supports that are unique to each person.  Through the combination of onsite staffing, technology, and remote supports, each person has the opportunity and autonomy to create a meaningful life that looks they way they want it to.  Join Ayelet during this session to learn more!

Join this workshop to learn from a group of people who use Supported Living Services! A dynamic group of people will answer questions and share their experiences living in their own home and receive supported living services.

Staff from Charles Lea Center, FIRST, & Liberty Corner Enterprises
Provider agencies identified as best-practice providers of Supported Living Services in NC will share their innovative and creative approaches to providing Supported Living in NC! Join this workshop to learn from DSP, QP, and Director perspectives on how Supported Living Services work best for the people they support (and staff!).
