Speaker: Kathryn Leach, DNP, CPNP-PC
Abstract: Children with medical complexity comprise a small proportion of the population, however they account for a large portion of health care expenditures. This presentation will explore best practices for improving health outcomes and reducing care giver burden. In addition, case studies will be presented to explore how care coordination can optimize health care utilize and enhance quality of life for patients and families.
Objectives:
- Define children with medical complexity and care coordination
- Classify and navigate the multi-faceted nature of care coordination and discharge planning and discuss best practices for how to communicate it to families
- Identify evidence-based strategies and tools for navigating multidisciplinary teams, enhancing communication, and addressing barriers to care for children with medical complexity

11:00AM – 12:00PM       Enhancing Pediatric and Family Patient Care Through Understanding Human                                                      Development, Cognition, and Health Literacy
Speaker: Audry S. Motlagh-Harvey, LCSW-C, MPH
Abstract: This session helps us understand how human development, cognitive processes, and health literacy principles can significantly improve pediatric and family patient care by tailoring communication strategies to age-appropriate levels, addressing cognitive limitations, and ensuring clear comprehension of health information, ultimately leading to better health outcomes and increased patient engagement across diverse developmental stages.
Objectives:
- Understand the importance of human development across the lifespan and be able to apply knowledge to direct practice with pediatric patients and their caretakers
- Review dimensions of cognition, how it changes across the lifespan, and influence on health behaviors and decision-making
- Define health literacy and understand the 3 dimensions of health literacy that impact health outcomes with the family system
- Identify validated tools for assessing cognition and health literacy in clinical setting and how to use results for clinical practice

1:15PM – 2:15PM:          Speed Learning Session and Interactive Panel Discussion
Speakers: Maryanne Bourque, MS, BSN, RN, CRCR, ACM-RN
Abstract: Similar to health care organizations serving other patient groups, Pediatric, Maternal/Child Health service providers face challenges meeting the demands of the evolving environment. This interactive session discusses workforce challenges including brainstorming common problems and solutions. Panelists provide a brief foundational overview. Moderator-led audience participation is encouraged to share best practices.
Objectives: 
- Discuss common challenges within Pediatric and Maternal/Child case management practice
- Describe impactful solutions
- Through audience participation, identify best practices and potential resources to implement solutions
